Registration

You can register in person at the surgery at any time during the surgery opening hours.

All you need to do is bring along your Medical Card (if you have one). If not, just come along and we will be able to provide you with a registration form to complete.

You will also be asked to complete a brief questionnaire in order to give the Doctors and idea of your previous medical history.

In addition, you will be given an appointment to come and see the doctor or nurse for a New Patient Check but only if you are on Repeat Medication.

New Patients

All newly registered patients over the age of 16 are invited to attend a health check at the surgery.

It gives you an opportunity to meet us and provides us with an overview of your medical history whist we await your notes to be forwarded from your last surgery.
