Obtain a Sick Certificate

Certificates for sickness for periods of less than 7 days should usually be signed by the patient themselves using form SC2 (available from your place of work). A medical certificate is not required. If your employer insists on having a medical certificate a fee will be charged.

If you are self employed or unemployed please ask the Receptionist for form SC1.
