Repeat Prescriptions

Patients on regular medications (drugs) will require repeat prescriptions. Each time a repeat prescription is written a new request slip is produced for you to make your next request for regular medications.
This can be handed in at the surgery.

Alternatively you can send a request:

	Via the Internet from our website: click here for details .
	Via post: please use your normal repeat slip and send to the surgery. (If you have mislaid the normal repeat slip, then please print out this form, fill it in and send to the surgery) .
	On your repeat slip please tick the medication you require and bring or send it to the surgery. If the medication you need is not on the slip then write on the item name, strength and daily dose.
	Remember to tell us where you wish to collect the prescription.
	Please give us plenty of notice, two full working days is the shortest time we need. If you can give us longer it would help us make sure your prescription is ready when you come to collect it.
	If you have to write the medication you require on your request slip then please try to give us more notice.
	Please write on your request slip when you need to collect your prescription by if you are giving us more than two full working days notice.
	Please read the repeat slip carefully. We sometimes put special information on it for you.
	We operate a dispensing service for patients registered at Bradninch and Uffculme.
	We are sorry but we cannot accept telephone prescription requests.
	If you require us to send your prescription back to you by post, please use the postal service and include a stamped addressed envelope.

PLEASE DO NOT USE THE INTERNET IF YOU REQUIRE YOUR PRESCRIPTION TO BE POSTED TO YOU.
