Repeat Prescriptions

Patients on regular medications (drugs) will require repeat prescriptions. Each time a repeat prescription is written a new request slip is produced for you to make your next request for regular medications.
This can be handed in at the surgery.

Alternatively you can send a request:

	Via the Internet from our website: click here for details .
	Via post: please use your normal repeat slip and send to the surgery. (If you have mislaid the normal repeat slip, then please print out this form, fill it in and send to the surgery) You need to enclose SAE if you wish to have it posted back.
	On your repeat slip please tick the medication you require and bring or send it to the surgery. If the medication you need is not on the slip then write on the item name, strength and daily dose.
	Please tick on the bottom of the repeat slip where you wish to collect the medication.
	We ask for, two full working days for repeat medication. If you can give us longer it would help us make sure your prescription is ready when you come to collect it.
	If you have to write the medication you require on your request slip then please try to give us more notice.
	Please read the repeat slip carefully. We sometimes put special information on it for you.
	We operate a dispensing service for patients registered at Bradninch and Uffculme.
	We are sorry but we cannot accept telephone prescription requests.
