The number of forms doctors are being
asked to complete is growing rapidly. In some cases someone
other than the Doctor can sign the form for you.
Passport applications
Pasport applications can be signed
by a number of different professional people. As such, we
no longer offer to sign these forms for patients.
Holiday cancellation
Holiday cancellation insurance does
not need a doctors letter - it needs an official claim form
obtainable from your insurers ( a doctor does need to fill
part of it in). A fee is charged for completion of private
certificates. Please ask at Reception for details.
Insurance claims forms
As with holiday cancellations, insurance
claims require you to get an official claim form from your
insurers. The doctor will complete part of it and a fee will
be charged.
